Once i will get a visa O for retirement (not O-A), and then get my one year extension for retirement, will i need to go out of Thailand every 3 months?

No. One year extension can continue to be extended for one year, Only have to make 90 day address reports to immigration if do not travel. And if you travel you must obtain a re-entry permit prior to leaving.

At most locations the answer is no, but ISTR there are one or two rogue offices that require the medical certificate. Off the top of my head, Sri Racha sounds familiar?
