SattaheapSea
is your "BAsed on" roughly equivalent to "depends on"? kuhn-yu-kub

What you suggest sounds more like when I am about to describe to someone about something in a casual way. I am looking for a more formal descriptive sentence that could be used in a instructional document from the government or the bank. I try to put together a sentence in Thai using ขึ้นอยู่กับ kuhn-yu-kub, but it doesn't sound correct, but I might be wrong.
SattaheapSea
I didn't know you are planning to write an official document. i admitted my words are informal. try this. it 's the best I can help you for now. I might think of something better and later I'd get back to you. or who has an advanced level of Thai. anyone? plain plang pai tam .....(object(s))..... เปลี่ยนแปลงไปตาม as if you would use your 2nd example. "The insurance fee is based on your age and medical condition" ค่าประกัน นั้น เปลี่ยนแปลงไปตาม สถานะของสุขภาพ และ อายุ

bannork
I think ขึ้นอยู่กับ is fine in the context as it means depends on, ie 'Will we play tennis tomorrow? 'it depends on the weather' , 'the tax you pay depends on your salary', is 'according to your salary.'- is based on your salary. เปลี่ยนแปลงไปตาม ' changes according to' is similar but doesn't have he same stress/ emphasis, root as based on.
