1. Does anyone have an example letter for requesting a third kru-sa-pah? Our school is very small and not sure what/how to do. A sample example would be fantastic.

 

2. I assume it gets turned in with the standard application krusapa application to BKK. Is this correct?

I don't think the number of waivers in the past is part of the application process. I'd just use the same letters used in the past. It's a standard format(in Thai) which I'm sure is available to any school, big or small.
