On my last visa run, my TM 30 receipt (ripped off bit at the bottom) was not returned to me, it was loose inside my passport, and when my passport was being stamped, immigration must have forgotten to put it back inside my passport, I did not notice this until I was home.

 

I am already registered with my local immigration office, and am still at the same address. Can I just request a new one?, what paperwork do I need to take?

What paperwork you need probably varies from one immigration office to the next. Some may want a police report for the lost document, which is a bit over the top in my opinion.
