Old form had the fee for an extension on it. The new form that they came out with in January does not have it.

And you said it Joe.......the fee for an 'extension', not a 'Visa'.

I was tongue in cheek, questioning the use of the terminology.

It's to prevent corruption.

The new form also states that handicapped patients or persons with disabilities do not need to attend in person.
