    Here's what I now heard from a doctor at Bandon (Koh Phangan) by email:  "... Previously last 2 years, our doctors used to do only physical examination in order to sign the medical certificate but it made our team not comfortable with that to sign the form with doubt. So team (doctors) discussed with the boss and came up with the package of this one - investigation for Syphilis and TB with blood and Xray. ..." 

     

    But after further discussion it turns out they are willing to skip the X-ray and instead do a blood test for TB.  That costs an additional B220 for total of 650+220 = 870.  Pricey but in my mind worth it since they're already doing the blood test for syphilis and it avoids the radiation exposure of an Xray.

    Try another hospital or even a private clinic? (Samui Home Clinic Bangrak. I do not know if Dr Bew is allowed to do these but a quick question to her will tell you. Google is your friend.)

    People say it now has to be a hospital, not a clinic.  And on the Immigration Office's list of required documents it says "medical certificate (hospital only valid  for 7 days)".

    What kind of form have you seen more recently?

    They checked blood pressure and pulse, and asked for my weight and height (I knew it well, so no need to jump on the scale).

    No syphilis check on my health certificate, that is (to my knowledge) needed, when applying for B-extension.

     

    And this is the (my) recent certificate from Samui International Hospital, accepted by Samui Immigration...

    I see yours does not mention syphilis (I did an app photo translation) but it does mention TB.  I guess your hospital is willing to certify no TB based just on listening to lungs rather than X-ray.  I suspect Immigration doesn’t mind that but Bandon Hospital is either trying to be more professional or is trying to justify charging more by doing an X-ray.

    Did you have to get an X-ray? Now I asked them and they say yes it’s needed even for retirement extension. Attached is the form they say Immigration needs.

Just dropped by Bandon hospital in Koh Phangan.  They’re up to speed on the certificate.  B650.  But I was surprised they insist on not only a blood test (syphillis) but a chest X-ray.   They say the certificate has to say I don’t have TB.  Seems crazy, especially since BKK Immigration doesn’t even require a medical certificate at all.  Why the heck should I need that radiation exposure ...

    It's hard to understand why they would ask for bank documents of any kind for a 90 day report.  It's not required by law and not in other offices (at least not in BKK) and it makes no sense because when you report online or by mail you never include bank documents.   Have other people been asked for them?

    Years ago Samui Immigration only accepted medical certificate from governments Samui Nathon Hospital which involved both a blood test and a urine test.

    Nowadays they gladly accept medical certificate from private hospitals, I and most others use Thai Inter in Chaweng where you get the certificate within 20-30 minutes to the price of 250 baht.

    You declare yourself fit to stay and the doctor listens to your lungs. That's all.

    Thanks, good to hear.  Any chance you know of a good hospital for this on Koh Phangan (that's where I'm located)?

    Good advice but the problem is their list is not nearly as specific  as (from what I hear) the office actually demands.  For example this quibbling about bank books and the exact format of copies. 
     

    Also,  the list asks simply for a medical certificate without saying what it should contain and apparently now clinics are not acceptable.  I’ve even heard that only public (not private) hospitals qualify. Is that true? Some of the best hospitals in the country are privately owned.

    When I asked why, I was told, " Boss says have to have old bank book because you used money in bank method last year."

     

    I was still doing the same this year. So a trip home was required.

     

    I don't know if it would make any  difference, but maybe next time I will ask for a "last twelve months statement" from the bank.  Can't be any more expensive than a three month one.

     

    To be clear, are you saying they want a bank book that shows the entire 12 months since your last extension?  If that's the case, I'd like to know what they would do if your current book only goes back say 6 months, and you don't have the old one because you either lost it or discarded it.  There's no law that says you have to keep old bank books, and how could you know it's required if it's your first time with this office? (Other offices don't require this).  Samui's official document list just requires "6. Copies of a bank book, showing the all page." (sic).   Nothing about old books. Surely if you have a statement that goes back 12 months it should suffice .... ?

    Thanks - did they make you give them a copy of the owner's ID?  (Above I noted that may be impossible for me).

  11. Eva's list above (from May) includes "4. Print copies of your Rental agreement, House book & Landlords ID."

    My landlord is a large company (based in BKK) and I don't see how I could get an ID (I suspect there are multiple owners and anyway I have no contact with them, just the local manager).  I do have the lease and house book.  Do I really need the ID?

    Great idea.  I'm on Koh Phangan and will be going to Samui soon to apply.  So I can't get the forms in advance. Would you mind posting your pdf versions (the blank ones of course) here so I and others could download them? ????

Please update re bank documents (for retirement extension in Samui, using the B800K deposit method).  I know we need a copy of a bank book showing the required deposits throughout the year and also a bank letter saying we have B800K as of the date (or shortly before) of applying for the extension.  Questions:

     

    1.  How many days ahead of applying can the bank letter be dated?

    2.  What about an additional bank statement for the entire preceding year, as some offices are apparently requiring.  Does Samui require it now?

    When you relocated to a different province, you would however be faced with the problem that any in person applications need to be done at the IO where you are officially registered which might be inconvenient.

    In my case I have just moved to a new province and so a TM30 will be filed with the new address.   My next 90 day report is due in February (as is my OA retirement extension).  Since the TM30 will put my new address in the system, the question is whether the online 90 day reporting app will reject my next report because it's never seen a 90 day report with that new address. If so, I'll have to file it in person when I go to do my extension but it would be more convenient if I could take care of it online.  That way I could go earlier to do my extension since they allow more lead time for that.

    That's interesting about the reset, yes BKK is not like that.  About online:  I've been doing that in BKK, but I think I've heard that if you try to do an online 90 day report from a new address, it doesn't work.  You have to do an in-person 90 day report with the new address, then it's in the system and you can do it online after that.  Is that correct?

    Thanks, what about the 90 day in-person report?  In BKK could come 15 days early until 7 days late.  Same in Samui?  I'd like to do both that and the extension in one visit.

    Thanks for the suggestion.   Any idea how the Surat Thani office compares with Samui? Do they too require a lease and a return trip to pick up passport, etc?
