In 3 years I have often flew back to UK to renew as a tourist. Aproximately 8 times back. Recently I did my first 30 day extension. I am now getting a Non O as a parent to thai child. 

 

Getting this non O in laos; vientiane.

 

I downloaded the app form directly from their site but the very bottom of page is missing. Tried it twice same issue. Am I correct in thinking the form has to be issued by the embassy you are applying to?

 

The London consulate has their address as do vientiane etc. Has anyone got a link to the form? Maybe it was the printing shop I used twice?

 

 

That is a problem with form as posted on their website. I have not seen a form to download that is complete. It is the same as the old form other than the parts that are in red.

You can get one at the embassy before applying.

I have been hoping that somebody could get one and do a good color scan of it and post it here.

Ok so don't complete the form in advance complete at office. Thanks. So the form they provide is correct and does not have same issue and if so would you like me to get a copy and email it to tv or load it up or something?

The form they have available at the embassy is the correct and complete form.

If you can scan it it would help a lot. Then send it to me by personal messenger.
