Requirements are the same. At Suvarnabhumi they will do the form, photo and copies for a service charge of 200 baht above the 3800 baht fee for a multiple re-entry permit.

Yes

Passport photo page, visa, entry stamp, extension of stay stamp (if you have one) and TM6 departure card.
