hi, on filling out this form , am not sure what to put where is says, (under my passport number)

ISSUES AT? would this be the uk issueing  office in the UK> ie CARDIFF. next,, visaed at the royal thai embassy or consulate. , would this be where i got my visa from in the uk (once again CARDIFF ) thanks  KC

Why are you completing a TM18. It is a request for a new residence certificate for those with permanent residency. If that is what you want then it would be where your passport was issued.

Are you wanting a certificate of residence to get a driver's license, register a vehicle and etc.  This is the form for requesting that. https://www.immigration.go.th/download/1486548833812.doc

thanks for that MOD, looks like i was misinformed. , am after a motorcycle licence, perhaps i can also ask you this,can i just roll up with my UK full bike licence or is it a must  to have an international driving permit, i live in JOMTIEN, have heard conflicting stories on this

Your UK drivers license will be accepted. There is no requirement for it to be a international license.
