I used information from this forum and the Thai Embassy London website to prepare documents for my OA visa application this November. I thought it might be useful to post what documents were actually accepted for my successful application.

My background is that I am a UK citizen (and I have a verifiable address in the U.K.) but have been living and working as a teacher in Phuket for the past 18 months. I finished teaching and needed a new type of visa to continue to live here. I chose to apply for the OA visa in London as I was already intending to return to the U.K. to visit friends a family for a few weeks.

What documents were accepted?

One copy of Criminal background check (link on Embassy website followed) £45

One copy Medical Certificate obtained in Phuket (250 baht)

Financial evidence of 65000 baht per month.

One copy of my last three months of U.K. bank statements showing the deposit of rental income from the U.K.

One copy of a letter from my bank confirming it was my account etc

One copy of the U.K. rental contract

Three visa application forms

Three OA long stay extra information forms

Three passport photographs

All copies were signed/ authenticated by a solicitor (£45)

I applied in person on Thursday 9th November (queued maybe 1 hour) and collected the following day (waited about 10 mins)

All in all once all documents had been obtained a very straightforward process, and I now have nearly 2 years without having to worry about visas!

I had prepared 3 copies of everything just in case but only 1 was asked for and accepted. The website states 2 visa forms, photos not 3, however it was easy to quickly fill out an extra set of forms and I had a spare photo.

Congratulations.  I had a similar type experience at the Los Angeles Thai Consulate for the O-A.  I dropped off the application packet on a Wednesday at 1030 and picked up the Passport with the Ext of Stay on Thursday at 1030.
