My last rotation at work took me to Addis Ababa then Bangkok on my flight home. I arrived at immigration to be told I had to complete a Medical Check Form as I arrived from an African Country. This meant going out of immigration to the Medical Check Area. The airline staff never made us aware of this whilst on board. I have been working in Africa for 5yrs and have never had to complete this form before. It's probably due to a connection through Dubai which made the IO think this was my Departure Point.

 

Do we have a PDF of the Medical Check Form available on Thai Visa so I can complete this before my arrival in BKK. The queue was pretty long the last time and I would like to get through this line quickly if possible. I would have taken a few copies the last time but I got the last one.

 

Fortunately the IO the last time told me to come directly to him after completing the form so managed to avoid the Immigration Queue for a second time.

I have never seen the form. Certainly no download of it on the forum that I am aware of.

Edit: Found a list of a countries that are required to undergo "Health Control" on entry.
