Rental contract AND TM-30 required for ret visa renew?

Chiang Mai immigration has started requiring TM-30s for retirement visa renewals (and many other services).

My question: If I get my landlord to file a proper TM-30, do I also need to provide a copy (or original?) of my apartment lease contract? Or is the TM-30 all they need?
