December 19, 20178 yr Attached are copies of the auxiliary forms, handed out at the Jomtien immigration office information desk, when you apply to extend a retirement visa. They must be completed and accompany your TM7, application to extend your stay in the Kingdom, based on retirement. If you want to speedup your application process, print and complete them, before getting into the queue. You may notice an error on the second page, but I would not be so bold as to correct an immigration official form. IMG.pdf IMG_0001.pdf

December 19, 20178 yr Seems straightforward.. reason for stay is retirement and you accept the penalties for overstay which I remember doing at least once before.
February 5, 20188 yr I was at Jomtien this morning-5 Feb 2018- for retirement visa extension and was handed the forms both at the ticketing counter and at the extension desk which I completed within a minute or so and despite it being a Monday there was no one ahead of me in the queue. Total time in the place, about 5 minutes. I return tomorrow to collect my passport. Well done to Jomtien Immigration for treating those on retirement extensions speedily!
