Hi all,

 

this is my first post. I went to Nonthaburi Immigration to get a 30 extension on a tourist visa and was given the following form. I have no idea what it and cannot find any information about it. I would greatly appreciate any help. Thanks.

It's a form for Thai people to register family member who isforeigner. (It's what it's written on the title)

Without any TM number, i think it's an 'officemade' paper.

This is actually a "guarantee" by the Thai family that they will provide a place to live to the "foreigner". Further, there is no agreed upon price for rent. Immigration is looking for a promise from the Thai family that they will provide you with a place to live while you are in Thailand.

Yeh as mentioned above. Its a form for Thai requesting the id card number and everything to be signed by the Thai person who provides the residence as a guarentee. Other bottom 2 signature line are for other residents or dependants. You perhaps.
