? about TM7 Form

I have a question about the line on the form that asks about "kind of visa". She got a 30 day exemption stamp when she arrived in BKK and wants to extend for another 30 days. I am just not sure what to put and any examples I found on the net didn't help. I have also read that the copies of her arrival stamp, passport info page and TM6 need to be signed and have a phone number. Some posts say yes, others have not said anything about it. I just want to make sure the paperwork is filled out properly. Appreciate any insights. Thanks.

The form says type of visa. Put tourist or visa exempt.

All copies must be signed by the applicant. No need for a phone number on them.
