All the major hospitals can do it. For reasonable cost, could consider Phayathai 2 Hospital, which also has an IVF center should that prove necessary.

The cost depend on the tests that the doctor will orden to both members of the couple. Sorry, I don't know the cost of those test in Thailand, only the cost of IVF treatment. You can call and ask them to have an idea.
