Confusion with filling TM8

I'm applying for re-entry but am confused what I full in here.

I am on retirement extension, so do i fill in Non-immigrant and the date of the original visa or the date of my latest extension?

The part you've highlighted in red is asking about when you will return. You've cropped the form too much as you enter the return date on the next line. Just put an approximate date. They don't hold you to any dates. The re-entry permit will be valid for entry until your current permit to stay expires.

 

They want the original visa information, not the extension of stay.

Not really, as there is another space for the date. Thanks for the confirmation about the visa info.

You don’t need to put anything in the space after “and shall return”, just a date in the next line.
