Next week I am applying for a Non-Immigrant O Visa in Savannakhet.  On the application it asks for the name, address and telephone number of Guarantor in Lao.  I do not know anyone living in Lao.  What is typically done for this application.  Can I just leave this blank?  Thanks all.  This is where I obtained the application form; http://www.thaisavannakhet.com/images/savannakhet/25571020 Visa Form/SVK-VisaForm.PDF

Never filled that one in, and got visa every time.

You can leave the it blank. it is is only for Laotians when they apply for a visa.

Is it only 1 frontside for savanakhet application, nothing in the backside? Mean we can print and bring this one page only? Thanks.

Do you hand over copy of the wifes name change document with the application as well? I can't remember what I did last time.

The marriage cert has her maiden name but id card and passport her married name.

That is not required.
