I used Bangkok hospital. Very modern and efficient. As I reacall it came to 5,000 USD.

Sorry i am not in C.M but read this as i am getting mine done ,5000 dollars? did you buy the hospital as well ????????

even in BKK Pattaya hospital which is the most expensive dont charge that much .

Greenside .Use the search function lengthy discussion just a 3 months ago ? Mine cost around 48,000 baht per eye, 1 yr ago. Siriphat Hospital  Dr Vinai. Highly recommended
