Need help translating Thai address into English

I would put some on the same line that it would look about the following

 

Bunsuan Thongkham

134 Mu 1 Nanoy

Nan 55150

Thailand

 

The "Tambun" and "Amphoe" Nanoy I would write only once, as very often you not have the space for so much writting.
