Is any application form needed for 2nd consecutive 90 day report

You don't apply to do a report of staying longer than 90 consecutive days in the country. The form used is a TM47 report form.

At most offices you do not need anything more than your passport and the report slip from your previous report. At the most you would only need a completed TM47 form if the office you report to requires it.

Bangkok, the largest immigration office, requires a new TM47 be filled out and submitted for every report (as of 4 months ago)  Also of a report 4 months ago they also wanted a cell phone number be entered in the telephone area (regardless of having landline).

For Jomtien, only needed passport with previous report slip (last week).

 

In June this year, reporting 90 days after returning from an overseas trip, the IO just binned my new TM47 form and stated only need passport and new TM6.
