When does health insurance become mandatory also will I need it before my extension of stay is for renewal in september

Title appropriate

You will not need it for an extension of stay, it is only mandatory for Non Immigrant O-A nothing else. and startdate is something like 1st july 2019.

Plse feel free to correct me guys..

It doesn't. At the moment they are only proposing (not yet confirmed) health insurance for Non-Immigrant 'O-A' visa applications from Embassies.
