If applying for a re-entry permit the same day as being issued a Non-O Retirement visa (converted from SETV) how would you fill out the "My Previous Visa" section of the form?

 

I am thinking enter new Non-O dates, but use the last date I entered Thailand.

You would use the date your non immigrant visa was issued and they did the new entry stamp showing the new 90 entry you will get.

I assume you are aware the re-entry permit will only be valid until the day the 90 days end.

I return tomorrow to CW for my stamp, so I will get a new entry stamp with the visa and post that on the request?

 

PS Yes on the valid dates.

It is simply a Non-Imm-O entry, not yet a retirement extension. Permission will be 90 days and Visa type Non-Imm-O.
