I got a 1 year multi entry visa based on marriage at Savannakhet. My 90 days will soon be up and i need to go to my local immigration and ask for a 60 day extension.

What documents do i need to take and is there a form i need to fill out ? (or anything else i need to know?)

You wife will need to be with you when you apply.

Marriage certificate and a copy of it, copies of your wife's house book registry and ID. Proof of residence to confirm you live where the office is located. 

You need a completed T7 application form with a 4 X 6 photo attached.

You may need to submit a TM30 form if one has not been done for where you are living now.
