Which date of entry to use on 90 day report form?

Simple question here. The 90 day report form has a field "Entered Thailand on".  For an non-immigrant O visa on extension for a few years, should the date entered here be the original date of entry into Thailand from a few years back, or the most recent entry on a re-entry permit?

It is your most recent entry to the country. I your case that would be one from your re-entry permit.

That used to confuse me too. Im glad Im not the only one. The form is not clear.

 

To be honest for a few renewals I put the original entry and I got the renewal no problem but I was doing it wrong.  

 

I was eventually told by a mate to use the date on latest entry ..which was far easier to look up.
