but how to fill in the present address?

If I go to penang to make tourist visa,

Should I write some hotel name address?

How to write the Date of previous visits to thailand ?

Is that the date of last entrance date to thailand?

or last exit date of thailand?

Which should I write the date previous visitsto thailand as 2APRIL or 7April?

in the past (9 years ago) under 'previous visits' I entered: "many, last: date in - date out"

for current address I wrote the hotel address I stayed at.
