I'm on the second year of an original O-A visa issued in 2019. My permission to stay expires on March 10, 2021. I'm going to apply for an Extension of Stay Based on Retirement. I plan to go to Immigration on Friday Feb 26. When I buy the insurance (now!), do I need the start date to be March 10 2021 or February 26 2021?

the insurance has to be valid on the day you go to immigration, and that will be the day your permission to stay will start from

You will not be able to do the extension on the 26th since it is a holiday and immigration will be closed.

What day the insurance starts can depend upon the office you go to. Some will accept insurance for the day your current permit to stay ends or will want it be in effect on the day you apply.
