Question regard to the Medical Certificate Covid-19 free, to enter in Thailand

Hello, in this certificate, (Medical Certificate for General Passenger)

 

it said specify the date :

 

What date should be included, the date of the blood was taken, or the date of the results ?

 

In the past 14 days with evidence of negative testing with the COVID-19 RT-PCR test not more than 72 hours before departure (specify date ……………………………………………………………………………………………………) << ??

I was told to use the date of the test, and NOT the date of the result, I got my result back in 24hours so not  problem for me

 

Remember you will need a fit to fly cert also
