COVID vaccine passport at Pattayarak Health Unit

Just want to share my experience regarding getting the COVID vaccine passport via walk-in.  We arrived at Pattayarak Health Unit in Na Kluea a little bit before 8:30am (the time their office opens).  It was quiet; there were no other applicants for the vaccine passport except us.  The staff told us to take a seat.  Shortly after, the staff told us to go to Window 1 and submit our documents (i.e. a copy of our passport photo page and a copy of our vaccine certificate issued by the hospital which vaccinated us).  They told us to sign our names and write down our phone numbers.  After that, they gave us our queue tickets.  In about 15 mins, we collected our vaccine passports and paid 50 bhat each.  The entire process was very smooth and quick.

That's good to hear. My wife has made us an appointment for the 15th to do this but in BKK.

The clinic is right behind the fish market.  Google Map will take you to the clinic based on the address from your screenshot above.  Below is a nicer picture of the building.  It is facing the water side with a nice park in front.  The clinic also has Facebook presence.  Here is the link:  https://www.facebook.com/ศูนย์พัทยารักษ์-111234613913576/
