I got mine at a Krungthai bank.

There are usually M Pass offices next to the Bangkok toll booths. 

Pattaya used to have a little M Pass shack next to it's toll booth but it's long gone. 

There's an office at the Mabprachan 2 rest area on the 7 going to Bangkok on the outskirts of Pattaya, in the Pong area. 

Got one at the Krungthai Bank in Little Walk. Had to wait more than an hour but got there in the end.

They really should make it a lot easier and more convenient to get those passes. And there's no reason for the credit on the passes to have an expiration date.
