HIV surge exposes Myawaddy’s health crisis

HIV infections are rising sharply in Myawaddy, the border town between Myanmar and Thailand that has become notorious for Chinese‑run scam syndicates and illicit nightlife venues. Civil health organisations warn that prevention efforts are paralysed, with authorities reluctant to act against brothels, karaoke bars and massage parlours linked to the military and allied armed groups.

Colonel Soe Moe Win, Karen State’s Minister for Security and Border Affairs, recently told parliament that 25 inspections this year had found no illegal activity. Local health workers dispute this, insisting that more than 100 venues are operating openly, many with infected individuals who are not receiving treatment. “The official figures do not match the reality,” one volunteer said.

The Irrawaddy reported in July that over 2,600 patients are currently receiving HIV treatment in the town, up from 1,688 in 2024. Transmission is largely driven by sex work, compounded by drug use and the influx of foreign workers tied to scam operations. Health groups stress that mandatory medical testing for sex workers is essential, but say they are barred from entering venues controlled by militia groups.

Residents describe official prevention campaigns as little more than public relations exercises. Recent education talks were limited to five locations, far from the high‑risk areas where infections are spreading. “It feels like they are just doing it for show,” one local said, pointing to the absence of testing in karaoke rooms and drug‑use “Hi” rooms.

Myawaddy is under the control of the Myanmar military and its allies, including the Karen Border Guard Force and the Democratic Karen Benevolent Army. Their involvement in the local economy makes shutting down prostitution politically sensitive. Colonel Soe Moe Win claimed plans to combat sex work have been submitted to township authorities, but arrests would depend on “specific information”.

The steady rise in HIV cases highlights a dangerous gap between official statements and the situation on the ground. With infections climbing year on year, health organisations warn that without decisive action, Myawaddy risks becoming a “black hole” for HIV control in the region. For locals, the fear is clear: the town’s reputation as a hub for scams and drugs is now being matched by a growing public health emergency.
