I stopped at Phuket Immigration today to pick up the PR application forms etc so I can start preparing for my application for next year, but all they gave me was the T.M. 9.

I didn't get the sample of medical certificate etc. Does anyone know of a copy of the sample medical certificate somewhere on the Internet?

Is it an actual form from immigration that the hospital has to complete? If not, will the hospital know what's required and just do their own thing or would I have to show them the sample medical certificate?

I don't want to go all the way to Bangkok just to collect the missing paperwork.

Hospitals will have copies of required government medical forms that they can fill out - I don't know the specific form you need but suspect it is probably among there collection.

You should be able to get an answer by calling 1111 hotline and asking for Immigration and then asking the officer if it is a specific form or the general work permit type medical.
