Zebra crossings with lights work, at least the one in Hua Hin, across from Bangkok Hospital, does. In general, sans lights, your best to just ignore them as any indication that you have a right of way as a pedestrian; occasionally, a vehicle, or vehicles, will stop to let people cross, especially if their elderly or infirm in some way, but it's very hit or miss.
