I am an American and I just got a 1 year type "O" Non Immigrant mult entry Visa for Thailand. I will be arriving on Sept. 5th and I will be staying in Chiang Mai.

Do I have to leave the Country every 90 days or can I just report to immigration at Chiang Mai?

You have to leave.

Yes have to exit/re enter every 90 days . Do your last border run just before the Visa expires and you get another 90 days making a total of 15 months.

Most people in your neck of the woods do a 'border run' to Mae Sai/Tachilek. It can be done in 1 day.
