I have seen the example form on the site, but it's not the same as mine.

What should I fill out here? -->

"Name and Adress of Local Guarantor"

"Countries for which travel document is valid"

And can I request for more than 1 entry?

Here is the application form i have to fill out. -- couldn't add the link?

What visa are you applying for, and where are you now?

i'm in norway, applying for normal tourist visa.

This is a multi-visa application form. Country visiting complete as Thailand. Guarantor details box you can leave blank, as only applicable for non-immigrant visas. You can apply for more than a single entry tourist visa at your embassy/consulate, but single entry tourist visas (60 days) are currently free and can be extended for a further period of 30 days at Thai immigration.

I would put "self" for guaranter, or just leave blank.

Your passport is valid for all countries unless there is an amendment saying otherwise.

It is up to Consulate if they will issue more than one free visa but you can ask.
