I did it on a motorcycle recently. From what I have read it is no different for a car. First you'll need to get your book translated into English at any Land Transport Department in Thailand before your trip. They took 2 days to do mine. About 200 baht if I recall. Then as soon as you cross the boarder at Sadao, turn in the completed "temporary vehicle export" form attached below (if I can find it) at the Thai customs window. Presumably if they decide to check out the car when you return they will have the record of you leaving. In practice it seems this step can be skipped.

After you do your passport stuff on Thai and Malay side, about 200m past the Malay immigration is row of shops on the right side of the road opposite the tourist information center which offers temporary auto insurance. The minimum is 30 days. They will also make the English letter stickers to put on the back of the car. Then you take the insurance and translated book to another building next door and get a certificate which makes you legal on Malay roads. You may need to park your car where they can see the English letter translation of your license plate on the back of the car. Also, if you have tinted windows, roll them down so it's not obvious. They may take issue with that.

Having said all that, I know people that did none of this and just drove right through the boarder on a motorcycle with no problems. They were only required to get their passport stamped in and out, but the motorcycle was ignored, and they never had any problems in Malaysia. They would probably have been in trouble if stopped bu the police with no insurance or translated book and plate.

Even doing it by the book it's all pretty painless as there is always someone around which will be pointing you in the right direction.
