I am assured by Bangkok Bank that I can use my ATM card in Italy to withdraw Euros at a good exchange rate. The card is a Be1st Visa which I've used to buy air tickets and fund PayPal. Neither my BKK nor my SCB cards worked in the US. Anyone have any info? Thanks.

I have Siam commercial ATM that works in UK and Greece

I have a Be1st Bangkok Bank Visa card, and I have used it to withdraw money in Germany and the US, no problems.

I have used my Kasikornbank atm card in the UK and France, no problems, have also used it throughout SE Asia and Australia.

I think Thai banks offer domestic and international cards, that's what my bank told me. Have had no problems using a Thai card in UK-but have had problems using it at my own Bank in Bkk!

Before going to Malaysia, I had to exchange my Katsikorn ATM card for a different one which can be used abroad. In Malaysia I could indeed use it, but not at all ATM's.

I never had problems with any Bangkok Bank, Kasikorn, and SCB debit card in the US, UK, or Japan.

Getting a "good" exchange rate is highly subjective. "Good" could mean anywhere from 1-3% off the Interbank Rate. In my experience, I wouldn't withdrawal large amounts of foreign currency using an ATM while outside of Thailand using a Thai ATM card. Remember you will also get hit with the 100 baht fee as well. :)
