The Thai Marriage Visa requires a document from the U.S. embassy proving "Evidence of Income" .... I did a quick search of the U.S. Bangkok Embassy site but didn't find anything.

Anyone know what is required to get this document? Tax records, bank account statements, pay stubs?

There is no evidence needed.

You fill a form in stating your income and the official signs it.

All, you do is attest to the amount that you fill in on the paperwork and pay the fee takes about 10min.. No additional paper work is required at this time

Yup, you swear (affirm an oath) under penalty of perjury that the information provided on the income form is true and correct. No documents required.
