Going to KL Malaysia this Tuesday with my thai wife, to apply for my first Thai Non-O Marriage Visa Multiple entry

Just have a few questions I'm stuck on about filling out the VISA form, my first time doing so.

-"Number of entries Requested" _M_ ?

-"Current Address____"

home address in the USA?

-I first arrived on Oct 2 on a 30 day then went to Cambodia on the Oct 29 and received a 14 day visa.

What date should I put on the form for my "Date of Arrival in Thailand___"? Oct 2 or Oct 29?

-"Traveling By____" I assume by "airplane"

- "Flight or Vessel Name___" should this be my Original flight into Thailand on Cathay Pacific, or my Air Asia Flight from Malaysia??

-"Duration of Stay 90 days " correct?

-"Date of Previous Visit__"

Visited Thailand on Aug 28 for 3 weeks

or

my current visit before VISA run to Cambodia on Oct 2

-"Purpose of Visit :"

"Other visit wife or family " ? or ?
