"Getting out of dodge" fall-out from a failed realtionship - Guide for dummies.
Well thats a little harsh. There are quite a few of us who have been with a Thai woman for decades. Probably not from the entertainment biz, although I do know some of those who have gone the distance. But that being said, the landscape is strewn with bad Thai/Farang relationships. Bargirls, guys twice. the girls age, language, culture the list goes on how it can fail. But thats the story of human life, relationships fail, maybe in the West for different reasons, but know when to cut bait, pack what you need and leave. Always tough to leave something you think you've built, but you live and move on -

Vertigo
Look up the description for Benign Paroxysmal Positional Vertigo (BPPV) and see if it matches your symptoms. @Kinnock gave some details about it. There are several different flavors of the problem depending on what part of the inner ear the small crystals occupy. It is called 'Positional' because certain head movements and directions of head tilt will cause the 'Paroxysmal Vertigo' sensations. There is no forever cure for it but as Kinnock mentioned there is a specific serious of head motions that will often move the crystals in the inner ear to an area where they no longer cause vertigo. Look on YouTube for the Epley maneuver or search for BPPV. My wife suffers from it. In her case the vertigo occurs if she is not careful when she tilts her head forward and downwards. If she quickly stops the movement at the first sensation she is often OK after a few minutes of sitting still and looking straight ahead and focusing far away. -

What are my visa options?
That's good news. For non O retirement annual extensions there are two methods. Money in bank (800k) OR income method. Being Dutch you can use income method. Is the 70k per month that you mentioned a pension? Since you dont have a Thai bank account you best option is to obtain the Non O retirement eVisa outside of Thailand. BTW: The Thai gf doesn't come into the equation for immigration purposes. What's with the 6 months in Cambodia -

How much do you pay for health insurance?
You get it working in Thailand. Just use the government hospitals, many of them are great, and they like paying customers. Where are you? I see my liver doctor at Chula, and it is pretty cheap.
