I have 1 year teaching contract with school. Now I want to apply for non -immigrant B Visa.

The School just give me documents. I would like to know about visa forms filling.

1) Should I write there 1 year or 3 months?

2) Should I apply for Single entry or Multiple Entries?

3) After getting Non-Immigrant B visa, What is procedure of extensions? Could I get Extensions at bkk or to go out from thailand and re-enter?

If you qualify for one year extension of stay (it appears you will) you want a single entry non immigrant B visa for a stay of 90 days. You would then apply at the immigration office serving your area for a one year extension of stay during the last 30 days with a TM.7/photo/school paperwork and 1,900 baht.

I am going to apply for new Visa Non Immigrant B ( first time) at bangkok, which forms I should use

1) Application for new Visa

2) Application for temporary extenion of stay in Thailand

Did the school say you could do this? Normally believe teachers have to exit to a nearby Consulate to obtain the visa. Enter on that and after work permit is issued and time down to 30 days apply for extension of stay.
