Requirements For Initial Retirement On Non O Multi Entry

I know this has probably been done many times before, but will someone please list the requirements needed for an initial (first time) application at Chang Wattana for a retirement extension based on:

1. With 800K Thai Baht in a Thai Bank (seasoned over 3 months)

2. Over 50 years old

3. Living in Bangkok with valid U.S. passport and current valid entry on a Non O multi entry visa.

4. Not married, but can provide proof of Thai address if required.

In short I would like a checklist of what Chang Wattana expects to see and I should have for that iniial application for the first 90 day report.

I thought that list was pinned somewhere but now I can't find such a list.

1. Bank passbook/copies with 800k or more on deposit 2 months (first application) and bank letter of same amount in account on same day (or obtain proof of account balance on same day with transaction) if letter previous day (this is not set in stone but many reports of people having to go downstairs to update. There are branch banks on ground floor and can be used.

2. Passport/copy date page-visa page-entry and arrival card.

3. TM.7 application for extension filled out with 4x6cm photo taken within last 6 months attached.

4. Fee of 1,900 baht.

5. You should have some form of address proof (as that determines where you apply) and be able to draw a map to home if asked.

6. You will be asked to fill out several one page forms/sign at desk. All copies should be signed and your phone number should be entered at top rear of TM.7 form.
