I remember long ago at Suanpluu, we had to photocopy some pages of our passport. Does anyone know the exact pages that we have to photocopy ?

I kind of need a pdf version where the Thai scripts are intact. The format in the Word version contains plenty of check boxes with no Thai scripts.

The word version is fine for me - are you using word to open it?

Yes, I use Mac version of Word.
