Re-Entry Permit Form Tm 8

For a few years I've been using the TM8 form available from this forum - http://www.thaivisa....plication-form/

Yesterday at Ranong Immigration they did not accept this form as it was only one page whereas the one they had was two pages (single sheet - front and back). Despite being exactly the same information spread over two pages I had to complete their form.

Does someone have a two page version in a similar format as the above download.

p.s. I did check the Immigration Bureau site and their downloads (in both doc and pdf formats) are the one page versions - http://www.immigrati...p?page=download (but as the link suggests the site has not been updated in 8 years)

Whilst not exactly the same as the form Ranong Immigration gave me (their form does not have the Duty Stamp box) hopefully with a bit of editing it will do.
