My wife cleverly photocopied what appear to be the relevant pages:

The first page with parents, house, then the page with her remarks on it.

The copy two pages are on one composite page.

No, only to be married and to obtain cert and KR2. Ihad read all we actually need is my pp and her id - just covering all the bases so to state.
