Tourist Visa Application Form Question, Passport On Maiden Name

I and my wife will have a ten day vacation in Thailand in the first half of December. We want to apply for tourist visa at the embassy, but my wife’s passport and all her travel documents (flight tickets, hotel vouchers) are made with her maiden name. The visa application form has the following fields:

First Name Middle Name Family Name

and on the next line:

Former Name (if any)

We are stuck here. How do we fill Family Name and Former Name? If we fill the “Family Name” with her actual (married) family name that means that the visa applicant person (full name is on the first line on visa application form) is different from the person in the passport.

If you fill the “Family Name” with he maiden name (as in the passport) that means lying I guess, and there is nothing to fill at the former name field (and she obviously had one). Furthermore, there is another field called Marital Status which should be filled Married (for not lying), so I guess we should fill both “Family Name” and “Former Name”.

The embassy is also requiring a bank letter which is also issued with her married name (not maiden name as in passport). Will this be a problem?

What country is your home country? Do you need a Visa?

The name on the visa application form must be the same as in the passport. Depending on where you apply for the visa, it will be in the form of an adhesive label and the applicant's name will be printed on the visa.

coming for a 10 days visit to Thailand... why make so much fuzz about that, just come, and get the ordinary 30 days or 21 days..... But maybe you come from a country outside "the list" of normal countries that have free time here for holidays...

TIT - LOL, even on that official List, the Entries of the recently, upgraded countries from Visa on Arrival,

to Visa Exemption countries have not been deleted from their old status and have now double entries on the lists.wink.png

Yes, that is the question, maybe the OP, -bolovan- knows already very well, that he needs a Visa.
