Surat Thani province

Suratthani office has no proper queing system. When you arrive there could be literally hundreds of Burmese waiting to be dealt with. Just ignore all of these and get yourself into the office. The lady you need to see is 2nd desk on the left as you go in the door. Good luck!

There's also an immigrations office on Koh Samui that's part of Suratthani.

I think Suratthani is the only province with two offices, correct me if I'm wrong.

Chonburi has 2.
