There is no from, the procedure will be up to your Immigrations office.

If you are not on extension of stay take with you proof of residence (rent contract).

Some offices will charge you some not.

Can take half an hour up to two weeks depending on office.

Do the same rules apply to owning a motorbike?
What documents do Immigration need to give me a certificate of residence, and how long it is valid ?

I just got a letter from my embassy confirming my address, did have to pay for that though, garage did all the rest.
