Just needing to affirm what I think is right on 90 day reporting

I have been in Thailand nearly two years but will travel to USA for the first time since coming here October 1st 2011. I originally had an O-A Non-Immigrant Visa which is expired and now on a one year extention of stay which expires October 12th 2013. I also have a multiple re-entry stamp that also expires October 12th 2013. My next 90 day report is due on September 2nd, 2013. I will fly from Bangkok to Indianapolis Indiana August 27th and return to Bangkok on September 17th and plan to shortly after go to get another one year extension of stay around October 1st, 2013. I believe I am correct in assuming that I will not need to do my 90 day report since I am leaving Thailand before the due of September 2nd, 2013 and if you could just re-affirm that I would appriciate it. Also I would like to know if I should fill out my current departure card (which is blank now) before I arrive at the airport when I leave on August 27th. I know this topic is beat to death already but since it is my first time traveling since coming here I just wanted to make sure I am doing everything correctly. Thank you in advance and thank you Thai Visa Forum.

Yes you are fine - no report of address until 90 days after your return.

I do not have a current departure card (mine is the old card stock) but never had to fill anything out on that other than name/passport information which I always did when arriving.
