Just confirming that this is correct since i read a thread from 2011 saying i had to go to Suay something something (cant seem to find the right search string to give me the thread again ). And if i get it right, need photocopy of passport, proof of address (hotel im staying at okay?) and picture (oh and 1900 baht).
